Instructions for Completing the Loss/Damage Claim Form

Terms and Conditions for submitting a claim

1. Claim Filing Requirements

A claim will not be considered properly presented until ITI has received a completed and signed claim form indicating a demand for a specific amount of money. 

 

Claim Filing Deadlines: 

  • Regular household goods shipments: within 45 days of delivery
  • All other shipments: within 48 hours of delivery

(Refer to your insurance certificate if you are unsure which deadline applies.) 

 

Preservation of Damaged Goods: The container and contents should be preserved in the condition received until the survey or inspection is completed unless further damage would result. The insurance company reserves the right to inspect or verify all reported damages and may request substantiation of claimed amounts. Do not dispose of or transport damaged items without written approval from I.T.I. or until your claim is finalized.

 

 

2. Filing a Claim Online  

(You will need your insurance certificate to login to the website.   If you do not have your insurance certificate, please contact the mover who arranged your insurance.)  

 

Step 1 of 2 – Required Information:

  • Email address
  • Street address, city, state/country, zip code
  • Phone and alternate phone
  • Pack date, pick-up date, delivery date
  • Packed by, unpacked by
  • Storage status and details
  • Other insurance information
  • Signature
  • Additional comments

Step 2 of 2 – Itemized Claim Details:

  • Inventory or carton number
  • Location listed on valued inventory
  • Description of each item and type of damage
  • Age, acquisition date, original cost, amount claimed
  • Currency type being claimed
  • Additional comments

3. Salvage

If a replacement or total loss payment is made, Underwriters may claim salvage rights.

 

4. Damage Documentation

You must provide:

  • Photographs of the damage
  • Repair estimates must be provided from a professional repair firm that is qualified to repair the items in question.
Repairs must not be made without written authorization from ITI.
(Estimate and appraisal fees are not covered.)

 

5. Carrier Notification

Notify the air/ocean carrier and the origin/destination agents immediately unless your mover has already done so. Send copies of all correspondence to ITI.
